Who we are, and what we do.

 

What is a Primary Care Network?

Since the NHS was created 70 years ago, the population has grown - and we are living longer. Many, however, are living with long-term physical and mental health conditions that mean they need to access their local health services more often.

To meet the needs of our changing population, GP practices work together with community, mental health, social care, pharmacy, hospital and voluntary services in their local areas in groups of practices known as Primary Care Networks - or PCNs.

PCNs build on existing services, allowing for more proactive, personalised, better coordinated and more integrated health and social care for people closer to home.

Vale of Evesham PCN is committed to the development and delivery of coordinated healthcare, working with partner organisations to support patients in making informed decisions about their personal care – promoting health and wellbeing and ensuring equality of access for all.

What does that mean?

It means providing innovative services that are designed around the needs of our community—such as social prescribing and first contact physiotherapy.

How do you do it?

We work collaboratively with local organisations in the healthcare and voluntary sectors, as well as in partnership across GP practices. This helps us to ensure we’re providing a comprehensive, joined-up service.

Why?

Because together, we’re stronger. Working collaboratively means we’re supported by our colleagues across our network—so our patients can be better supported by us.

Meet the Team

  • Dr. Chethan Reddy

    CLINICAL DIRECTOR

    Chethan Reddy is a GP partner at Merstow Green Medical Practice and has worked in Evesham area since 2013. He qualified in India and moved to UK, initially training in Paediatrics and later completing his GP training in South Worcester in 2007. He represented The Vale(EBBI) in the South Worcester Alliance Board and was involved in creation of Neighbourhood teams as part of transformation programme. He has been leading the Neighbourhood team since 2018 and the Care Home project working with NT and practices.

  • Dr. Neill Bramble

    CLINICAL DIRECTOR

    Dr Bramble has been a GP in Evesham since 2015 and is a partner at Riverside Surgery. He trained in Birmingham before completing his GP training in South Worcester. Prior to this he was a Medical Officer in the British Army and deployed to Iraq and Afghanistan. As a result, Dr Bramble enjoys working as part of a diverse and skilled team and is determined to develop the PCN to benefit patient care and the staff working within it. He takes specific responsibility for the social prescribing element of the service.

  • Kirsty Taylor

    PCN Operations Manager

    Kirsty has worked in primary care now since 2023, first starting as Deputy Operations Manager and now stepping up to Operations Manager. Prior to working within the NHS Kirsty has experience in senior leadership within theatre arts and high street retail.

    Kirsty’s role within the PCN is to coordinate the smooth running of an increasingly diverse team of clinical staff and staff allied healthcare. She holds all line management responsibilities for the PCN and uses her experience within multiple industries to help develop additional services to the benefit of patients.
